2. Health and measurement notes

Please share only health information that may affect the measurements or your comfort, such as a relevant condition, medication, recent injury or surgery. You may leave this blank and speak with us privately instead.

3. What the assessment involves

NT Body Lab uses ISAK anthropometric measurement methods. Some measurements include skinfolds and body dimensions, and require us to touch your skin or position it at measurement sites. We will explain what we are about to measure. You may ask questions, request an adjustment, or ask us to pause or stop at any time.

We will take and record body measurements and results for your assessment and follow-up. We will discuss your results with you. The results are for your assessment record and follow-up, not for public sharing or marketing unless you separately consent to photo or video use below.

4. Before your assessment: how to prepare

Please come fasted and hydrated. Fluids are encouraged as normal. Please be advised that these measurements require having access to sites on your torso and limbs, access to skin. Women: please wear a sports bra or halter top and loose or compression shorts. Men: compression shorts are ideal. Parking is available at Lionheart Fitness & Tanning, 127 Sowers St, State College. Check in with the front desk to receive a free parking pass.

5. Optional photo or video during the assessment

Photos or video, if you agree, are for your assessment record only. This choice does not affect the service.

6. Separate optional permission for marketing

This is a separate choice from allowing a photo or video in your assessment record. We will not use your photo or video in advertising or marketing unless you say yes below. Your answer does not affect the service. If you leave this blank, no marketing permission is given.

7. Optional temporary audio recording

We may use a temporary audio recording only as an aid to enter your assessment data. It will not be used for marketing or shared. We will delete the recording right after it has been transcribed.

8. Your information and your choices

Your completed form and assessment records are stored in Wix as part of NT Body Lab's records. Our team can access them through Nittany Table's Wix account. Other collaborators with relevant access to that account may also be able to access them. The information is used to prepare for, conduct, record, and follow up on your assessment. Your assessment data will also be shared with the International Society for the Advancement of Kinanthropometry (ISAK) and added to the ISAK database (isakmetry), as part of the larger world population data set. To ask about your information held by ISAK, contact info@isak.global. Your information is not used for marketing unless you separately consent to photo or video marketing above.

You may withdraw your consent at any time by telling us. We will stop the measurement or recording that you ask us to stop. If you withdraw consent after information has already been collected or shared, talk with us about what can still be changed.

9. Your signature

10. Parent or guardian consent for a client under 18

If the client named above is under 18, a parent or legal guardian must review and sign this section before the assessment.
